When making your appointment, please remember to provide the following information:
- Your health insurance information and photo ID
- Your home address and telephone number
- The name of your primary care doctor, and/or the name of the doctor who referred you to SDIPCAC
When you arrive for your appointment, please remember to bring:
- Your insurance card, photo ID, and necessary referral information
- Chest x-ray, CT scan images, pulmonary function test, laboratory tests and any other appropriate clinical data
- Medication list

Directions from the North
Take the 5, 805, or 15 South to the 8 East. Exit College Avenue, turn right. Turn right onto University Avenue. The hospital is on the right.
Directions from the East
Take the 8 West. Exit College Avenue, turn left. Turn right onto University Avenue. The hospital is on the right.
Directions from the South
Take the 15 or 805 North to the 94 West. Exit Euclid North. Euclid becomes 54th Street. Turn right onto University Avenue. The hospital is on the left.
Directions from the West
Take the 8 East. Exit College Avenue, turn right. Turn right onto University Avenue. The hospital is on the right.
SDIPCAC is above the street level up a long driveway. Look for the large Promise Hospital sign & American flag.
